Create Your Patient Portal Account
You will receive an email to create your AthenaHealth portal account and complete your Health History Form.
The AthenaHealth portal is where you can access information about your visits, request prescription refills and more.

Please arrive 10-15 minutes early to complete the necessary paperwork, which may include initiating your medical record request form, consent forms, etc).
